Please provide the names of all other members in the party who will be staying in the house. Only those named are allowed to stay in the property.

Requests for changes to the booking form must be sent in writing.
Please send the completed form to : Mr & Mrs K H Worthen, Sanderling, The Street, Mickle Trafford, CHESTER CH2 4EP, United Kingdom enclosing a cheque for the deposit (£150 / $200 per week). Please make cheques payable to : KH & EE Worthen.
The balance of payment and security deposit are due two calendar months prior to arrival date.
Confirmation of the booking will be sent, directions and access details will be forwarded when the full payment and security deposit have been received.
